Superannuation Funds

If necessary, a patient may choose to apply for funds through their Superannuation fund. In my practice there have been no instances of rejection. It can save a lot of time to make enquiries prior to consultation. We still recommend patients check with their fund or join prior to surgery. The Cutoff date for payment is 2 weeks prior *to surgery or the procedure is cancelled. We don’t run a paying off system. Patients can if they choose to use a Money Lender but mostly they join health funds or are assisted by a generous relative.

To Claim Superannuation a completed Superannuation form from General Practitioner and DR Paul Caska is required. The Form is "Early Release of Superannuation on Specified Compassionate Grounds" and is available from the Department of Human Services www,humanservies,gov.au/earlysuper or call 1300131060

A written Quote from Dr Paul Caska / Anaesthetist / Hospital. WE have had no instances of rejection.

It can save a lot of time to do this before the consultation so please advise the secretary when you call to make your appointment.

The Item Numbers are Gastric Band Item #31569 Gastric Sleeve 31575 Cutoff date for payment is 2 weeks prior *to surgery or the procedure is cancelled.
